According to experts, Staphylococcus aureus is the most common culprit behind this infection. However, other bacteria can also be responsible, depending on the source of infection, the patient's age, and overall health status. The manifestation and affected bones of this infection vary across different life stages.

Osteomyelitis in Newborns and Infants

In newborns, osteomyelitis often affects long bones like the femur and tibia, and multiple bones can be infected simultaneously. Acute hematogenous osteomyelitis is most common in this age group, where the infection reaches the bone via the bloodstream.

Besides Staphylococcus aureus, Group B Streptococcus and Escherichia coli are also significant pathogens in newborns. In infants and toddlers, the infection typically focuses on the metaphyseal regions of long bones, particularly the femur, tibia, and humerus. Acute hematogenous osteomyelitis remains prevalent here, with Staphylococcus aureus being the dominant bacterium, though Kingella kingae and Streptococcus pyogenes can also cause infection.

Osteomyelitis in Children, Adolescents, and Young Adults

The pattern of infection shifts slightly in adolescents and young adults. In this age group, the pelvis and spine can also be affected, in addition to long bones. Infections can develop following injuries, open fractures, or severe accidents.

Staphylococcus aureus, particularly MRSA (Methicillin-resistant Staphylococcus aureus), is a major cause of infection in this age group. While rare, other bacteria can also be responsible for the disease.

Here, alongside Staphylococcus aureus, Gram-negative bacilli become significant contributing factors to infection. In individuals over 65, vertebral osteomyelitis (spine infection), diabetic foot osteomyelitis, and infections related to prosthetic joints (e.g., artificial hips or knees) are more prevalent.

In this age group, Gram-negative bacteria and polymicrobial infections (caused by more than one bacterium) are common, in addition to Staphylococcus aureus. Advancing age, a weakened immune system, and other chronic illnesses further increase the risk of these infections.

Types and Risk Factors of Osteomyelitis

Osteomyelitis presents in several distinct forms. Acute hematogenous osteomyelitis primarily affects children. In contiguous osteomyelitis, the infection spreads to the bone from surrounding soft tissues or wounds.

Post-traumatic osteomyelitis develops after an open fracture or significant injury, while postoperative osteomyelitis can occur after orthopedic surgery or implant fixation. Long-standing infections are termed chronic osteomyelitis, characterized by the formation of dead bone fragments (sequestrum) and new bone (involucrum).

Osteomyelitis is found to be nearly twice as common in men as in women. This is largely attributed to a higher incidence of open fractures, workplace injuries, accidents, and orthopedic surgeries in men. However, in cases like diabetic foot and prosthetic joint infections, both sexes can be equally affected, with risk being more dependent on individual health conditions.

Key risk factors for osteomyelitis include diabetes mellitus, open fractures, orthopedic implants, joint replacements, sickle cell disease, weakened immune systems, intravenous drug use, peripheral vascular disease, and chronic kidney disease.
