In these situations, cold compress or cryotherapy (ice application) is recognized as an effective and safe treatment method during the first 24 to 72 hours after an injury. It not only reduces pain and swelling but also proves helpful in preventing further damage to the affected tissues.
The fundamental principle of cryotherapy is to lower the temperature of the affected area, thereby controlling the body's inflammatory and pain response. Exposure to cold slows down nerve activity, reducing the transmission of pain signals to the brain. Simultaneously, blood vessels constrict, which helps control excessive blood accumulation and swelling (edema) at the injury site.
Cold compress also plays a significant role in limiting internal bleeding and protecting against secondary tissue damage after an injury. It reduces muscle spasms, providing immediate relief and facilitating the patient's prompt initiation of the rehabilitation process.
Application of Cold Compress in Various Injuries
In soft tissue injuries, which include damage to muscles and subcutaneous tissues, cold compress is particularly useful for reducing pain, swelling, and bruising. It is widely used for common sports injuries.
For ligament injuries or sprains, especially ankle and knee sprains, ice therapy aids in controlling swelling and maintaining joint functionality. Cryotherapy is also effective in relieving pain and swelling during the acute phase of tendon-related injuries, such as tendinitis or tendon strains.

When using cryotherapy, certain specific precautions must be observed. Applying ice directly to the skin can cause frostbite or severe skin damage. Prolonged continuous application of ice packs should also be avoided.
Patients with circulatory problems, peripheral vascular disease, or extreme sensitivity to cold should use it only after medical advice.
Heat Therapy and When to Switch
Generally, after the initial 48 to 72 hours post-injury, when swelling and acute pain begin to subside, heat therapy (warm compress) can be more beneficial in many situations. Warm compresses increase blood flow to the affected area, reduce muscle stiffness, and help accelerate the healing process.
However, it is crucial to avoid warm compresses in cases of active inflammation or acute pain, as this can worsen the condition. Experts opine that cryotherapy is a simple, safe, and scientifically proven primary treatment method for acute orthopedic injuries.
When adopted in conjunction with proper immobilization, compression, elevation, and timely medical evaluation, it effectively controls pain and swelling, reduces tissue damage, and significantly aids the victim in returning to normal activities sooner.
