The Bennett fracture, occurring at the base of the first metacarpal bone and extending to the first carpometacarpal (CMC) joint, is a complex injury. This type of fracture necessitates not only bone healing but also the critical maintenance of joint stability.
The CMC joint of the thumb is vital for daily activities like grasping, lifting, and pinching. Therefore, its treatment prioritizes restoring the correct joint position alongside bone union. This injury typically occurs when sudden, intense pressure is applied to a bent thumb.
Common causes include the body's weight falling onto the hand and thumb during a fall, a direct blow to the thumb, or excessive force applied to the thumb in an accident. Immediately after the injury, intense pain at the base of the thumb, swelling, and unbearable pain upon touching the first CMC joint may occur. Difficulty in moving the thumb, grasping objects, or pinching is experienced. In some cases, joint instability or a visible deformity at the base of the thumb can also be present.
Fracture Characteristics and Diagnosis
The defining characteristic of a Bennett fracture is its specific fracture pattern. A small volar-ulnar fragment of the base of the first metacarpal bone, attached to the trapezium bone by the anterior oblique ligament, may remain in its position. Meanwhile, the rest of the first metacarpal bone shifts posteriorly, radially (towards the thumb), and superiorly due to the pull of the extrinsic thumb muscles, particularly the abductor pollicis longus.
This muscular pull causes the CMC joint to subluxate (partially dislocate) or fully dislocate from its normal position. X-rays of the thumb and entire hand are required for diagnosis. Typically, anteroposterior (AP), lateral, and oblique views are taken.
These X-rays provide a detailed assessment of the fracture severity, the extent of bone fragment displacement, and the status of the CMC joint. If the fracture is particularly complex or the X-rays do not fully clarify the situation, doctors may recommend a CT scan, which offers 3D imaging of the bone and joint.
Treatment Modalities: From Non-Surgical to Surgical
The primary goal of treating a Bennett fracture is to return the first metacarpal bone and the CMC joint to their original anatomical position as much as possible and to maintain stability. If the fracture displacement is minimal and the joint is stable, non-surgical treatment may be an option.
This involves immobilizing the thumb using a special 'thumb-spica' splint or cast. Regular X-rays are taken to ensure the bone and joint remain in their correct position without undue movement. Surgical intervention becomes necessary when there is significant displacement of the bone fragments or joint instability.
In a 'closed reduction' procedure, attempts are made to realign the bone fragments and the joint without making an incision. Subsequently, the bone is stabilized through 'percutaneous K-wire fixation,' where thin wires are passed through the bone fragments to hold them together.
In some cases, if closed reduction is not possible, displacement is extensive, or the fracture has a complex articular pattern, 'open reduction and internal fixation' (ORIF) is recommended. This surgical procedure involves making an incision to access the fracture site, clearly visualize the bone fragments, and then permanently stabilize them using plates, screws, or other implants.
Recovery and Precautions
The key principle for effective treatment of a Bennett fracture is restoring the CMC joint to its correct anatomical position, repairing the joint surface as much as possible, and ensuring strong, stable fixation. Timely diagnosis and appropriate treatment can effectively preserve thumb grip strength, pinching ability, and overall hand function.
Ignoring persistent pain, unusual swelling, deformity, or reduced thumb mobility after an injury, considering it a minor issue, can be serious. Any such symptoms should be taken seriously, and immediate consultation with an orthopedic specialist is advised. Examination by a specialist and timely, correct treatment can prevent complications and largely restore thumb functionality.
