Subchondral Cysts: A Common Cause of Osteoarthritis and Their Diagnosis and Management
Subchondral cysts, also known as geodes, are fluid-filled cavities beneath cartilage, frequently linked to osteoarthritis and requiring specific diagnostic and management approaches.

Understanding Subchondral Cysts
Subchondral cysts, commonly referred to as geodes, are well-defined cavities that form in the subchondral bone, just beneath the articular cartilage within joints. These cysts typically contain fluid or fibrous tissue and are most frequently associated with osteoarthritis (OA).
While primarily linked to OA, these cysts can also appear in conditions such as rheumatoid arthritis or other inflammatory arthritides, osteonecrosis, and certain other joint diseases. On medical imaging, particularly X-rays, subchondral cysts often manifest as round or oval radiolucent (appearing relatively dark) lesions situated just below the joint surface, sometimes surrounded by a thin, dense (sclerotic) rim.
In cases of osteoarthritis, these geodes frequently coexist with other characteristic changes like subchondral sclerosis (bone hardening), osteophytes (bone spurs), and joint space narrowing. It is notable that these cysts tend to develop more prominently in areas of the joint that experience persistent, excessive mechanical loading.
Causes of Subchondral Cyst Development
In the context of osteoarthritis, the articular cartilage within the joint gradually deteriorates. This degenerative process leads to an increased mechanical stress on the underlying subchondral bone. Prolonged exposure to this elevated pressure can result in microfractures within the bone.
Under specific circumstances, synovial fluid from within the joint can penetrate these micro-cracks, entering the subchondral bone and eventually forming a cystic cavity. Concurrent with this process, reactive sclerosis can also develop in the bone surrounding the cyst. It is important to recognize that the precise mechanism of geode formation is complex and not fully explained by a single process; it likely involves multiple factors such as micro-damage, abnormalities in bone remodeling, and increased joint pressure.
Differential Diagnosis and the Role of Imaging
When subchondral cysts are detected, osteoarthritis is considered the most common underlying cause. However, clinicians must also consider other potential causes, including rheumatoid arthritis or other inflammatory arthropathies, subchondral insufficiency fractures, osteonecrosis (tissue death due to lack of blood supply to bone), and calcium pyrophosphate dihydrate (CPPD) crystal arthropathy.
If there is unusually rapid bone destruction, infection must be seriously considered. In younger patients, depending on the cyst's location and imaging characteristics, other types of cystic bone lesions should also be evaluated. Magnetic Resonance Imaging (MRI) offers crucial insights into the nature of geodes and the condition of surrounding tissues, revealing fluid signals within the cyst, edema in the adjacent bone marrow, and potential communication with the joint cavity.
MRI is also invaluable for assessing associated cartilage damage, meniscus or labrum tears, and the state of other joint structures. If cysts are not clearly visible on X-ray, physicians may order an MRI.
Treatment and Management Strategies
In most instances, small, asymptomatic subchondral cysts do not require specific treatment. The primary focus remains on managing the underlying joint condition, particularly osteoarthritis. For patients experiencing symptoms, various management strategies can be beneficial, including modifications to daily activities, strengthening muscles through physiotherapy, engaging in appropriate exercises, and weight reduction for overweight individuals.
Pain relievers or non-steroidal anti-inflammatory drugs (NSAIDs) may be used as medically indicated. In select cases, intra-articular injections may be administered, and associated cartilage or meniscal problems are treated. When osteoarthritis is severe, causing significant difficulty with mobility and daily activities due to persistent pain, joint replacement surgery may be considered after a thorough evaluation of the patient's individual condition.
It is important to note that subchondral cysts themselves are not tumors. Within the context of typical osteoarthritis, they are usually simply an imaging finding of degenerative joint disease. However, if cysts are unusually large, rapidly growing, or causing excessive pain, especially in the absence of typical osteoarthritis changes, further imaging (like MRI) and diagnostic investigations may be necessary to rule out other underlying issues.
