Understanding Femoroacetabular Impingement (FAI)
Femoroacetabular Impingement (FAI) is a condition affecting the hip joint where abnormal contact occurs between the femoral head-neck junction and the hip socket (acetabulum) during normal activities. Experts note this is a significant concern for young individuals and athletes involved in sports like football, hockey, dance, or martial arts.
Repeated collisions can gradually damage the hip's labrum and cartilage. This can lead to persistent pain, reduced hip function, and early-onset osteoarthritis. However, with timely and accurate diagnosis and treatment, this condition can be effectively managed.
Types of FAI
FAI primarily occurs in three forms: Cam impingement, where extra bone develops at the femoral head-neck junction; Pincer impingement, where extra bone along the rim of the acetabulum restricts normal hip movement; and Mixed impingement, a combination of both, which is the most common type.
Rising Risk in Youth and Athletes
This condition is more prevalent in young people and athletes engaging in sports that require frequent hip flexion, rotation, and high-impact activities. Sports such as football, hockey, dance, and martial arts increase the risk of FAI. Excessive stress on the hip during these activities, along with congenital or developmental abnormalities, can further enhance the likelihood of developing this condition.
Symptoms and Identification
The most prominent symptom of FAI is pain in the hip or upper thigh, often exacerbated by prolonged sitting, squatting, climbing stairs, running, or any athletic activity. Many patients also report hip stiffness, reduced range of motion, clicking or popping sensations within the joint, and sometimes a feeling of the hip locking.
During a physical examination, a positive FADIR (Flexion, Adduction, Internal Rotation) test is considered a significant indicator of FAI. This test helps detect pain or discomfort experienced during specific hip movements.
Diagnostic Process
An FAI diagnosis begins with a thorough clinical examination. Subsequently, X-rays of the pelvis (AP and Dunn or lateral views) are taken to assess bone structure. If a labral tear or damage to the articular cartilage is suspected, an MRI or MR Arthrogram is recommended.
MRI provides detailed information about the hip's soft tissues, such as the labrum and cartilage. A CT scan can also be useful for a deeper understanding of bone structure and for planning potential surgery.
Treatment Options
The treatment for FAI depends on the patient's age, symptom severity, and the extent of damage to the hip joint. For initial and mild cases, emphasis is placed on conservative (non-surgical) treatment. This includes avoiding activities that aggravate pain, taking non-steroidal anti-inflammatory drugs (NSAIDs), and undergoing physiotherapy to strengthen hip and core muscles.
Physiotherapy aims to provide support through exercises that avoid positions causing impingement. In select patients, ultrasound-guided intra-articular corticosteroid injections may be administered to reduce pain and inflammation.
Surgical Intervention
If conservative treatment proves ineffective or if there is significant structural damage to the joint, surgery via hip arthroscopy is recommended. This is a minimally invasive procedure where the inside of the joint is treated using specialized instruments through small incisions.
During surgery, procedures like femoral osteoplasty (bone removal) for Cam impingement, acetabuloplasty for Pincer impingement, repair or reconstruction of labral tears, and treatment of cartilage damage are performed.
Preventing Complications
Experts believe that untreated FAI can lead to severe long-term consequences. These include worsening acetabular labral tears, articular cartilage erosion, premature osteoarthritis of the hip joint, persistent pain, and significant limitations in daily activities. Therefore, chronic hip or groin pain in young people and athletes should not be dismissed as normal.
