A recent study published by researchers analysing data from the SMART2D cohort highlights significant dietary differences across clinical subtypes of Type 2 Diabetes (T2D).
This provides critical insights into personalized approaches for managing the disease. The SMART2D cohort included 1,007 adults with T2D, from a multi-ethnic Southeast Asian population.
The average age of participants was 61 years.
Three Distinct Dietary Patterns
Researchers identified three major dietary patterns using factor analysis of 46 food groups:
- “meat, fast food and eating out,”
- “sugar-laden food and drinks,” and
- “plant-based and dairy.”
Participants were classified based on their dominant dietary pattern. Clear distinctions were observed between the identified subtypes of T2D: mild age-related diabetes with insulin insufficiency (MARD-II), mild obesity-related diabetes (MOD), and severe insulin-resistant diabetes with relative insulin insufficiency (SIRD-RII).

In contrast, approximately 38% of participants with MOD and SIRD-RII favored diets high in sugar-laden foods and drinks. Around 31% of MOD and SIRD-RII participants consumed meat-heavy and fast-food-oriented diets.
Diet and Comorbidity Risk
Statistical analysis confirmed that MOD and SIRD-RII were positively associated with less healthy dietary patterns. They were inversely associated with plant-based diets compared to MARD-II.
The study also linked dietary patterns to varying risks of comorbidities within each subtype.
Diets high in sugar and processed foods, as well as frequent eating out and meat consumption, were associated with poorer health outcomes. These risks were particularly elevated among individuals with MOD and SIRD-RII.
Personalized Diabetes Management
The findings emphasize the heterogeneity of T2D. They suggest that dietary behaviors are not uniform across patient groups.
