It is essential to understand that steroids cannot repair damaged cartilage, ligaments, or bone; their primary function is solely to reduce inflammation and the resulting pain. Incorrect or excessive use can lead to serious side effects that cannot be ignored.

Common Uses and Benefits in Orthopedics

The most prevalent use of steroids in orthopedics is in the form of intra-articular injections given directly into joints. These injections have proven particularly beneficial for patients suffering from osteoarthritis, especially in knee, shoulder, and in some cases, hip joint problems.

They are also utilized in conditions such as rheumatoid arthritis, gout, pseudogout, and other inflammatory synovitis. Steroid injections rapidly reduce inflammation within the joint, providing immediate pain relief, thereby improving patient mobility and facilitating physiotherapy and exercise.

Effectiveness and Limitations of Injections

While steroid injections offer rapid relief, their effect is temporary, typically lasting from 4 to 12 weeks. It is important to note that this is not a permanent cure for arthritis but an effective method for managing pain and inflammation.

Repeated injections into the same joint should be avoided, as overuse increases the risk of cartilage damage. A general guideline suggests not administering more than three to four injections in any joint per year.

Use in Other Soft Tissue Problems

Besides joints, steroid injections have also been found effective in treating various soft tissue problems. These injections can help reduce nerve inflammation, alleviate pain, improve mobility, and in some cases, postpone the need for surgery.

Potential Side Effects and Risks of Steroids

Alongside the therapeutic benefits, steroids are associated with certain inherent risks and side effects. These include an increased risk of infection, temporary elevation in blood sugar levels, skin thinning or discoloration at the injection site, increased post-injection pain, tendon weakening, and cartilage damage from repeated use.

Furthermore, long-term high-dose systemic steroid use can lead to problems like osteoporosis (bone weakening) and, in severe cases, avascular necrosis of the femoral head (cessation of blood supply to the upper end of the thigh bone).

Which Patients Require Special Caution?

Steroid injections should be used with extreme caution or avoided altogether in patients with any infection in or around the joint, septic arthritis, uncontrolled bleeding disorders, known allergies to steroids, uncontrolled diabetes (high blood sugar), and weakened immune systems.

In these situations, the potential risks may significantly outweigh the benefits.

Holistic Approach and Future Direction

According to experts, corticosteroids should be viewed as a tool for effective inflammation management in orthopedics, rather than a treatment that regenerates damaged tissues. To ensure the success and safety of steroid treatment, it should be integrated with other treatment modalities.

These include physiotherapy, regular exercise, maintaining a healthy weight, other medications as prescribed by the doctor, and positive lifestyle changes. Ultimately, the safe and successful outcome of steroid use depends on the core principles of application: the right patient, the right dose, the right location, and the right time.
