If not diagnosed and treated promptly, this condition can cause permanent neurological damage, including paraplegia, by compressing the spinal cord or nerves. Spinal hematoma can affect individuals of any age, with varying causes and risk factors across different age groups.

Causes and Risk Factors Across Age Groups

In children, it may be linked to congenital bleeding disorders such as hemophilia, platelet deficiencies, leukemia, trauma, or congenital vascular malformations. For young adults, major causes include injuries, spinal surgery, vascular abnormalities, and the use of anticoagulant or antiplatelet drugs. Middle-aged individuals face increased risk from these medications, alongside kidney or liver dysfunction, hypertension, trauma, and procedures like spinal injections.

Elderly individuals are more vulnerable due to age-related increased bleeding risk, polypharmacy, reduced kidney function, and falls. Studies indicate that medication use, bleeding disorders, trauma, and other medical conditions are more significant risk factors than gender.

The Importance of Medication History

A comprehensive medication history from patients is crucial for physicians. This includes information on the use of direct oral anticoagulants (DOACs) like warfarin, apixaban, rivaroxaban, dabigatran, and edoxaban, as well as heparin medications such as unfractionated heparin and enoxaparin. The use of aspirin, clopidogrel, or a combination of both anticoagulant and antiplatelet drugs also needs to be disclosed.

A recent systematic review highlighted cases of spinal epidural hematoma associated with DOACs, characterized by sudden severe neck or back pain followed by rapidly worsening neurological symptoms, underscoring the need for extreme caution in those taking these medications.

Recognizing Symptoms and Immediate Response

The clinical progression of spinal hematoma typically begins with sudden, severe back pain. This can be followed by radicular pain (pain along a nerve pathway), sensory changes, and leg weakness. In severe cases, patients may become completely unable to walk, develop paraplegia, and experience significant problems with bladder or bowel control.

Alarmingly, these neurological deficits can deteriorate rapidly within minutes to hours. Therefore, dismissing such symptoms as common back pain, disc problems, or age-related spinal stenosis and waiting at home can be extremely risky. Immediate medical attention is paramount.

Diagnostic and Treatment Strategies

Spinal MRI is the most critical diagnostic tool for spinal hematoma, helping to accurately assess the hematoma's location, extent, and pressure on the spinal cord. Routine blood tests (CBC), platelet count, coagulation time (PT/INR, APTT), and evaluation of kidney and liver function are also performed. The type of anticoagulant medication the patient is taking and the time of their last dose play a vital role in treatment decisions.

The initial step in treatment involves medically supervised cessation of the implicated anticoagulant medication. Prompt consultation with a neurosurgeon or spine specialist is essential. If the bleeding is severe, strategies to reverse its effects based on the medication type are employed. For instance, Vitamin K and 4-factor PCC (prothrombin complex concentrate) may be used for severe bleeding associated with warfarin. Idarucizumab for dabigatran and andexanet alfa or PCC (according to local protocols) for certain Factor-Xa inhibitors can be considered. Protamine is also used situationally to counteract heparin's effects.

Surgical Intervention and Better Outcomes

If the hematoma is causing severe pressure on the spinal cord or if the neurological status is rapidly deteriorating, immediate surgical decompression and evacuation of the hematoma are required. However, it's important to understand that treatment is not uniform for all patients. In select cases with mild neurological symptoms, non-surgical management under close specialist supervision may be considered.

Available studies confirm that early identification and timely intervention for spinal hematoma lead to better neurological outcomes for patients. In summary, the most critical practical message is that individuals taking anticoagulant or antiplatelet drugs who experience sudden severe back pain with new numbness, weakness, difficulty walking, or loss of bladder/bowel control should consider it a potential emergency like spinal hematoma. Such a patient must be taken to the hospital immediately. In this situation, time is not just a factor in treatment; it becomes the most critical determinant of the patient's long-term neurological prognosis.
