Human rhinovirus, long dismissed as a harmless cause of the common cold, is now being recognised as a direct cause of adult pneumonia, according to new clinical research analysing thousands of hospital admissions. The findings overturn decades of medical assumption and could alter how doctors assess and manage serious respiratory illness.
The study shows that rhinovirus is not confined to the nose and throat. Instead, it can infect the lower lungs, triggering pneumonia severe enough to require hospital care.
A virus doctors often overlooked
Rhinovirus has traditionally been excluded when diagnosing adult pneumonia. Bacterial causes or viruses such as influenza and COVID-19 were prioritised, while rhinovirus was often seen as an incidental finding.
That view is now changing.
Researchers analysed over 9,500 hospitalised adults tested using RT-PCR between January 2020 and December 2023. Rhinovirus was detected in 4.6% of patients, a rate high enough to raise concern among infectious disease specialists.
Seasonal spikes were seen in late winter to early spring and again in autumn, matching known community circulation patterns.
Co-infections masked the real threat
Nearly 50% of rhinovirus-positive patients also carried bacterial infections or other respiratory viruses, complicating diagnosis. Experts say this overlap likely led clinicians to blame other pathogens while overlooking rhinovirus as a primary driver of lung infection.
According to infectious disease specialists involved in the research, this real-world complexity explains why rhinovirus-associated pneumonia has remained under-recognised for years.
