Obesity and prolonged engagement in physically demanding activities that place excessive stress on the hips also increase the risk by burdening the hip joint, thereby escalating the likelihood of developing OA.
Symptoms of Hip OA
The most common indication of hip OA is pain, typically felt in the groin area or the upper thigh. This discomfort often radiates downwards along the thigh and can sometimes extend to the knee. Activities such as walking, climbing stairs, prolonged standing, getting in and out of cars, or putting on shoes and socks can exacerbate this pain.

In severe cases, significant deformities in the joint structure may be visible. However, treatment decisions are not solely based on X-ray severity. The patient's pain level, the impact on daily activities, and quality of life are given greater importance.
It is crucial to remember that not all hip pain is due to OA. Conditions such as avascular necrosis of the femoral head, femoroacetabular impingement, acetabular labral tears, hip dysplasia, occult or stress fractures, inflammatory arthritis, and septic arthritis can also cause hip pain and must be considered in the differential diagnosis.
Furthermore, conditions like Greater Trochanteric Pain Syndrome (pain on the outer hip), iliopsoas tendinopathy or bursitis (inflammation of the hip flexor tendon or fluid sac), lumbar radiculopathy (nerve compression in the spine), and sacroiliac joint problems can mimic hip OA symptoms, necessitating careful examination for accurate diagnosis.
Treatment Options: From Exercise to Surgery
Treatment for hip OA typically begins with non-surgical measures. Educating the patient about the disease, its progression, and necessary lifestyle modifications is a vital first step. For overweight individuals, weight loss can significantly reduce the load on the hip joint, providing pain relief.
Physiotherapy incorporates exercises to strengthen hip muscles, especially the abductors, extensors, and core muscles, which helps support and stabilize the joint. When necessary, assistive devices like crutches or walkers may be recommended to aid mobility. Non-steroidal anti-inflammatory drugs (NSAIDs) or other painkillers may be prescribed by a doctor to manage pain.
In select patients, corticosteroid injections into the hip joint can offer temporary relief from pain and inflammation.
Total Hip Replacement: When Other Options Fail
When persistent severe pain and significant functional loss render a patient unable to perform routine daily activities, and non-operative treatments yield no benefit, Total Hip Arthroplasty (THA), or total hip replacement, is considered.
Total hip replacement is a highly effective treatment for patients whose hip OA has advanced to the point of experiencing constant pain, pain at rest, severe mobility limitations, and significant difficulty with daily activities. Advanced radiographic OA is also an indication.
However, for younger patients, particularly if the cartilage is relatively intact and a correctable structural issue exists, joint-preserving surgery may be an option. In such cases, it becomes crucial to identify and address labral pathology, FAI, dysplasia, and early cartilage damage. Prompt diagnosis and treatment of these problems can help prevent or delay the progression of OA and are vital in selecting the appropriate treatment pathway.
Overall, timely diagnosis of hip osteoarthritis and appropriate treatment based on disease severity are paramount for reducing a patient's pain, restoring function, and maintaining quality of life. In early to moderate stages, exercise, weight management, lifestyle changes, and medication can be effective. For advanced cases, where pain and functional loss severely impact life, total hip arthroplasty is a proven and effective treatment option.
