The review encompassed 5 systematic reviews with 8631 participants, and 28 randomized clinical trials with 4360 participants.

Pooled statistical analysis revealed that exercise had only small, short-term effects on knee osteoarthritis pain compared to placebo or no treatment.

Limited and Short-lived Benefits

The certainty of the evidence for knee osteoarthritis was deemed very low, with effects diminishing in larger or longer-term trials.

Evidence for hip osteoarthritis showed negligible effects, while hand osteoarthritis showed small effects.

Outcomes were comparable to patient education, manual therapy, painkillers, steroid or hyaluronic acid injections, and arthroscopy.

Comparing to Other Interventions

Single trials showed exercise to be less effective than knee bone remodeling surgery (osteotomy) and joint replacement in the long run.
