Other affected areas include the origin of the plantar fascia on the sole of the foot, the patellar and quadriceps tendons at the front of the knee, the medial and lateral epicondyles of the elbow (e.g., tennis elbow), the greater trochanter at the top of the hip, and the ischial tuberosity at the lower buttock.
Beyond these common areas, enthesitis can also cause inflammation at the sacroiliac joints in the pelvic region, and at the points where ribs attach to the sternum (breastbone) and vertebrae (spine). Due to these varied locations, some patients experience heel pain as their main symptom, while others may have predominant lower back pain, hip pain, or discomfort in other joints.
Therefore, symptoms should always be viewed as localized pain that can worsen with the use of the affected limb. It's crucial to understand that the pain experience can differ significantly based on the affected enthesis.

Differentiating between enthesitis and tendinitis or tendinopathy is crucial from a clinical perspective. Enthesitis primarily relates to inflammation at the tendon or ligament's attachment point to the bone. In contrast, tendinitis or tendinopathy involves issues within the tendon's tissues themselves, not at its bony insertion.
Enthesitis often presents with changes like bone erosion or new bone formation, whereas tendinopathy is mainly characterized by tendon thickening and degenerative changes. For example, enthesitis at the Achilles tendon's insertion on the heel bone and tendinopathy in the mid-portion of the Achilles tendon are entirely different conditions, even though both can cause pain. The causes, treatments, and management strategies for these two conditions also differ.
Therefore, accurate physical examination and interpretation of imaging tests by a specialist physician are necessary for a correct diagnosis. This distinction ensures the most effective treatment approach.
Diagnostic Process and Imaging Techniques
The diagnosis of enthesitis typically relies on a combination of patient-reported symptoms, physical examination by a physician, and, when necessary, imaging studies. During a physical examination, the physician will palpate the affected area to check for pain and swelling and assess the range of motion.
Imaging techniques play a vital role in confirming the diagnosis and understanding the condition's severity. Ultrasound (USG) is an accessible and effective tool that can detect entheseal thickening, hypoechogenicity (reduced reflection of sound waves), Doppler vascularity (increased blood vessels indicating inflammation), erosions (bone damage), and enthesophytes (new bone formation).
Magnetic Resonance Imaging (MRI) is particularly useful for identifying inflammation, edema (fluid accumulation), bone marrow edema, and bone erosions in surrounding tissues. X-rays, especially in cases of long-standing disease, can help visualize chronic changes like bone erosion and new bone formation.
Medical Evaluation and Treatment
If enthesitis is persistent, recurrent, or appears simultaneously in multiple body parts—especially around the Achilles tendon, plantar fascia, pelvis, and spine—a medical evaluation to rule out an underlying inflammatory disease is highly recommended. This is particularly important when enthesitis is accompanied by other symptoms such as inflammatory back pain (worse at night, relieved by rest), psoriasis (skin condition), eye inflammation (uveitis), or inflammatory bowel disease.
In such cases, seeking proper investigation and diagnosis from a qualified specialist physician is crucial. Effective treatment can only be determined once the correct cause is identified. The treatment aims to reduce inflammation, relieve pain, and restore function.
This may include medications (such as Non-Steroidal Anti-Inflammatory Drugs - NSAIDs, or specific anti-inflammatory drugs), physiotherapy, and in some cases, steroid injections or biologic agents, which have proven highly effective in treating inflammatory diseases.
