New research has uncovered a notable relationship between ABO blood groups and the risk of developing autoimmune liver diseases, indicating that a person’s inherited blood type may play a more significant role in liver immune disorders than previously understood.

The study, published in Frontiers in Medicine, analysed data from multiple medical centres to explore whether particular blood types predispose individuals to autoimmune hepatitis (AIH), primary biliary cholangitis (PBC), or primary sclerosing cholangitis (PSC). These conditions occur when the immune system mistakenly attacks liver tissue or bile ducts, leading to chronic inflammation and potential liver damage.


Key Findings: Types A and B Linked to Higher Risk, Type O Appears Protective

Researchers reported several clear trends:

  • Blood group A showed the strongest association with autoimmune hepatitis, suggesting higher susceptibility among individuals with this common blood type.

  • Blood group B was most closely linked to primary biliary cholangitis, a chronic disease affecting the bile ducts.

  • Blood group O appeared to confer a lower overall risk for several autoimmune liver diseases, echoing previous findings where type O has been connected to reduced risk in other autoimmune and inflammatory conditions.

The authors emphasised that while blood type alone cannot predict who will develop liver autoimmunity, the pattern was consistent across patient cohorts.


Possible Biological Explanation: How ABO Antigens Interact With Immunity

One proposed explanation lies in how ABO antigens influence immune-system behaviour. These antigens—molecules on the surface of red blood cells and certain other tissues—may affect:

  • immune tolerance,

  • antigen recognition pathways,

  • inflammatory signalling, and

  • how the body distinguishes between self and non-self tissue.

Subtle differences in these processes could make some individuals more prone to immune-mediated liver injury.


What This Means for Screening and Personalised Medicine

While the findings are not intended for standalone diagnostic use, the researchers note that blood group information could complement existing risk assessments, particularly for people with:

  • a family history of autoimmune liver disease,

  • coexisting autoimmune conditions, or

  • unexplained liver enzyme abnormalities.

In these cases, clinicians might consider closer monitoring or earlier specialist referral.


Next Steps: Broader Studies Needed

Autoimmune liver diseases remain relatively rare but can lead to severe complications, including cirrhosis and liver failure, if left unidentified. The study’s authors call for further research in diverse global populations to confirm the associations and to better understand the immunological pathways connecting the ABO system with liver autoimmunity.

Such work, they argue, could eventually contribute to more targeted prevention strategies, improved risk models, and refined approaches to personalised care.
