Cam Impingement: A Growing Cause of Hip Pain and Immobility
Cam impingement, a type of FAI, is increasingly causing hip pain and restricted movement due to bone spurs in the femur head.

Understanding Cam Impingement
Cam impingement, considered a primary type of femoroacetabular impingement (FAI), is emerging as a significant cause of hip joint pain and immobility. In this condition, the junction of the femur's head and neck develops extra bone, deviating from its normal round shape. This abnormal bone growth is known as a 'cam bump'.
This bony abnormality repeatedly collides with the rim of the acetabulum (the hip socket's cup-like portion) during normal hip movements. Over time, this friction damages the labrum (a ring of tissue that stabilizes the joint) and the cartilage within the joint. This collision is particularly pronounced when the hip is flexed, adducted (moved towards the body), and internally rotated.
During this three-dimensional movement, the cam prominence makes contact with the acetabular rim, placing abnormal stress on the hip joint. Initially, this can lead to labral injury or tears. If the problem persists, damage to the cartilage, including delamination or defects, becomes more likely. Chronic cam impingement significantly increases the risk of developing osteoarthritis in the hip, severely impacting quality of life.
Identifying Symptoms and Diagnosis
The most common and prominent symptom of cam impingement is a deep ache in the lower abdomen or hip. This pain often intensifies during specific activities such as squatting, running, cycling, or prolonged sitting. Many patients experience a reduction in their ability to internally rotate their hip, especially when the hip is already in a flexed position.
Some individuals may also feel clicking, catching, or locking sensations in their hip, which can indicate labral pathology. During a physical examination, a positive FADIR test (flexion, adduction, and internal rotation) is often observed, pointing towards this condition. However, it's crucial to note that a diagnosis is not solely based on this test; other diagnostic methods are employed.
Imaging and Diagnostics
Various imaging techniques play a vital role in evaluating cam impingement. X-rays, particularly AP pelvis and Dunn 45 or 90-degree lateral views, are helpful for a detailed assessment of the femoral head-neck junction's structure. Parameters like the alpha angle and head-neck offset on X-rays aid in understanding the cam morphology.
Magnetic Resonance Imaging (MRI) or MR arthrography are highly effective in identifying acetabular labral tears, cartilage damage, and subtle bone changes. Computed Tomography (CT) scans are especially useful for a detailed three-dimensional evaluation of cam morphology and for surgical planning.
Treatment Options: From Conservative to Surgical
For patients with mild symptoms or in the early stages, lifestyle modifications and physiotherapy are the initial recommendations. The primary goal of physiotherapy is to strengthen the hip-stabilizing muscles, such as abductors, external rotators, and core muscles, while improving overall hip function.
In appropriate cases, medications like non-steroidal anti-inflammatory drugs (NSAIDs) can be used under medical advice to manage pain. In select instances, image-guided intra-articular injections can also provide pain relief. If persistent pain and other symptoms remain despite the structural issue, hip arthroscopy (key-hole surgery) is considered for selected patients.
During this minimally invasive procedure, excess bone can be removed using techniques such as femoral osteoplasty or cam resection. If necessary, labral repair and treatment of acetabular cartilage-related issues are also performed during the same procedure.
Holistic Approach: The Key to Diagnosis and Treatment
It is critically important to understand that the presence of cam-like structures on an X-ray does not automatically confirm symptomatic FAI. Many individuals may have cam-like bone morphology without experiencing any symptoms and lead normal lives. Therefore, treatment decisions should not be based solely on imaging reports.
A correct and holistic evaluation must consider the severity of the patient's symptoms, the findings from the physical examination, information from imaging studies, and, most importantly, the actual condition of the labrum and cartilage. This comprehensive and integrated approach forms the cornerstone for accurate diagnosis and the most appropriate treatment plan for cam impingement, leading to effective relief and prevention of future serious problems.
