Air pollution has long been known to damage the lungs and heart, but new global research reveals its devastating reach extends to the brain. According to the State of Global Air 2025 report, air pollution was responsible for an estimated 626,000 dementia-related deaths in 2023 — roughly 29% of all global dementia deaths.

The study, prepared by the Health Effects Institute (HEI) in collaboration with the Institute for Health Metrics and Evaluation (IHME), marks the first time scientists have quantified the neurological toll of polluted air on a global scale. Researchers found that air pollution stripped humanity of 11.6 million healthy years of life, underscoring its role as a silent but escalating public health emergency.


How Polluted Air Damages the Brain

The report details how microscopic pollutants, particularly fine particulate matter (PM2.5), can bypass the body’s natural defenses. After entering the lungs, these particles travel through the bloodstream and reach the brain, where they trigger inflammation and oxidative stress.

Over time, this process contributes to neurodegeneration, heightening the risk of Alzheimer’s disease, vascular dementia, and mild cognitive impairment — conditions that gradually erode memory, reasoning, and independence.

Scientific evidence increasingly shows that exposure to polluted air affects not only older adults but also the developing brains of children and adolescents. Studies link chronic exposure to autism spectrum disorders, anxiety, and depression, suggesting that air pollution disrupts brain health across all age groups.


A Gendered Impact: Women Bear the Double Burden

The dementia crisis, the report notes, disproportionately affects women — both as patients and as caregivers. Women are statistically more likely to develop dementia and also form the majority of unpaid caregivers for those suffering from it.

This “double burden” deepens the social and economic strain of dementia, especially in low- and middle-income countries where health infrastructure is already overstretched. As dementia cases rise, the caregiving responsibilities often fall to women, compounding gender inequality in health outcomes and economic participation.


An Ageing World, Rising Risks

Globally, around 60 million people were living with dementia in 2021, with 10 million new cases each year. With populations ageing and pollution levels remaining dangerously high, experts warn that the crisis could accelerate.

Older adults are particularly vulnerable to long-term exposure to polluted air, especially in countries where access to healthcare and clean air initiatives remain limited.

The 2025 report found that 86% of all air pollution-related deaths were linked to noncommunicable diseases (NCDs) — including heart disease, diabetes, chronic obstructive pulmonary disease (COPD), and dementia. In total, 7.9 million global deaths in 2023 were attributed to air pollution — equivalent to one in every eight deaths worldwide.


South Asia and Africa Bear the Heaviest Burden

The findings reveal stark regional disparities. South Asia and parts of Africa remain the hardest hit, with India and China together accounting for more than half of all air pollution-related deaths.

These regions face a dangerous combination of dense populations, rapid industrialisation, and inadequate air quality controls. For millions of residents, the exposure is lifelong — from infancy to old age — magnifying both physical and cognitive risks.


A Call for Urgent Global Action

Health experts say the new data should serve as a wake-up call for policymakers. Reducing air pollution not only extends life expectancy but also cuts healthcare costs, slows cognitive decline, and supports climate goals.

“Many decision-makers themselves are at ages where these effects are more pronounced,” said Dr. Pallavi Pant, head of global initiatives at the Health Effects Institute. “Clean air action is essential to ensure good health and a better quality of life for all.”

Pant, who oversaw the report’s preparation, added: “The data highlight the vast and growing impact of poor air quality on billions of people, particularly in Asia and Africa. We hope this evidence strengthens the global demand for clean air and drives targeted action where it’s needed most.”
